We have an exceptional opportunity for a full time Mental Health Case Manager to join our healthcare team at Northwest Correctional Center located in Tiptonville, TN.

Hours: 8am-4pm

Qualifications of the Corizon Health Mental Health Case Manager

  • Bachelor's degree in Psychology, Social Work or related Mental Health field from an accredited college or university.
  • At least 1 year experience in providing social, psychological or correctional case management services.
  • Subject to initial and ongoing security clearance requirements.

Responsibilities of the Corizon Health Mental Health Case Manager

  • Provide case management services under the direction of mental health administrator/site leadership.
  • Assists with reviewing, assigning, transferring, and discharging patients from mental health unit.
  • Coordinates and provides continuity of mental health care for patients upon entry into correctional facility.
  • Coordinates provision of adequate level of care during incarceration.
  • Coordinates community service upon patient's release from correctional facility.
  • Develops and implements and effective method of coordinating with the corrections staff and transportation departments for transfers and movement of mentally ill patients.
  • Other duties as assigned by supervisor.
