We have an exceptional opportunity for a Chronic Care Registered Nurse (RN) to join our healthcare team at South Idaho Correctional Institution located in Kuna, ID.

Corizon Health is offering $10,000 in tuition reimbursement and Five weeks of paid time off for new full time employees!

Qualifications of the Corizon Health Chronic Care Registered Nurse (RN)

  • Graduate of an accredited School of Nursing
  • Licensed as a Registered Nurse in the state of employment.
  • Previous experience with chronic care patients or in correctional healthcare is required.
  • Possesses an active CPR certification.
  • Excellent organizational, people and computer skills.
  • Remains knowledgeable about specific state laws and regulations governing practice.
  • Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.

Responsibilities of the Corizon Health Chronic Care Registered Nurse (RN)

  • Coordinate the chronic care program for patients
  • Assess and treat all chronic care patients
  • Perform chart reviews and gather data
  • Report needed labs and diagnostics to Health Services Administrator
  • Review transfer screenings and intake logs for tracking of new chronic care patients
  • Work directly with staff RNs for continuity of care.
  • Assist with monthly charting, auditing, and clinical duties as needed.
  • Other duties as assigned by supervisor.
