We have an exceptional opportunity for a full time Discharge Planner to join our healthcare team at the Lansing Correctional Facility, located in Lansing, KS.

Qualifications of the Corizon Health Discharge Planner

  • A bachelor's degree in social work or closely related field.
  • One year of experience in the social services field. Applicants with experience in a health care setting will be given preference.
  • Valid state social work license.

Responsibilities of the Corizon Health Discharge Planner

  • Provides appropriate and timely discharge planning from the hospital, arrangements for follow-up health care, assessment of financial resources available for patients, and referrals to appropriate community resources as needed.
  • Provides discharge planning to patients and families, determining which hospital or community resources should be included in the patient's discharge plan, and requesting the determination of level of patient care for nursing home or group placement.
  • Accurately assesses patient's financial resources, makes referrals to community groups to provide assistance as needed, and coordinates transfers to extended care facilities (ECF), group care homes, or other acute hospitals as appropriate.
  • Documents discharge plan in medical record, completing final documentation within two working days of discharge.
  • Functions as a member of a multidisciplinary case management team.
  • Participates fully as a member of the social and pastoral services department.
  • Participates in the patient satisfaction survey.
  • Participates in continuing education programs to maintain skills and social work licensure.
